Horizontal saccades are normal. Vertical saccades (12 seconds) are slow and hypometric upward and downward. This indicates a lesion of the RIMLF.

Slowing of horizontal saccades indicates a pontine brainstem lesion in the ipsilateral paramedian pontine reticular formation (PPRF).
